I already have a passport, so I'm not sure this would ever be an issue
for me, but I could fill out that form, not only for my immediate
family but for all my aunts, uncles, cousins, etc..

Not that it would matter.  My son, who was born in Orlando, Florida to
two natural born US citizens, was denied his rights on the accusation
that he was not a citizen.  The government will deny travel or other
rights to whomever they decide is unworthy.

I tried to point this out to those who were calling for a crackdown on
immigrants and "terrorists".  The question is "who decides?".  It's a
very slippery slope.
