I use lists. I use Basecamphq.com. I always put in calendar entries so my
phone reminds me when things are due, or I have appointments.

Doesn't always work, as I still miss some things... but it helps. For some
reason too, if I play music, on a headset, I get more done faster.

Otherwise, brain wander isn't all bad :) You need to relax sometimes. :)
