I setup and used the Rogers (in Canada) version of Ring Central.  We had to
buy the phones, but other than that the monthly cost was low.  The phones
came pre-configured, and I found the support and web-portal for management
very good.  There were a number of various rules that you could set up for
when/when not to ring through to cell, or even have the both phones ring.
 I did a bit of looking and for us the Ring Central (Rogers version) fit
the bill.  Not overly expensive (we had 5 or 6 lines) (we also just got new
numbers, as we didn't have a number to move) and each phone had a direct
line as well as the extension number.  Oh, and if you had a voicemail left,
it converted to MP3 and emailed it to you.
