Presuming the epidemic goes unnoticed and people don't take counter 
measures.

Small pox infections go through bodily fluid contacts I think 
(including coughing, sneezing etc). So protection is relatively easy. I 
would be looking for something with a few more vectors, possibly 
something where animals can be carriers as well, or something that 
lives forever (like anthrax). In this scenario using something that 
wears off quickly is not necessary because occupation of US territory 
is not an issue.

<shrug>

Second year chemistry class here came with warnings that if you did 
something stupid, you would be producing nerve gas. I know several 
people that made TNT just for the heck of it. It is pretty easy to take 
down a significant part of the power grid by just driving a truck into 
the poles of an electricity line. Buy a gun, put on a white pointy hat 
and start shooting at a school playground with mostly black children. 
Repeat 3 times and watch an entire city go up in race riots. Load a 
truck with fertilizer, drive into a tunnel and press the red button. 
Load a speedboat with explosives and some mercury switches and steer it 
into the side of an LNG tanker. All of these are much easier to 
organize as the use of biological agents.

Do all of these together, and the US has other things on its mind as 
invading Iraq or not. The potential for destruction and terrorism is 
still going up, despite the warnings. The world aint seen nothing yet.
