If, as it now appears, the left wing damaged on launch was the point of 
failure, looks like this one may yet again be caused by the engineers on 
the ground not acknowledging the seriousness of a problem.

In this case, as with Challenger, blame can be placed solidly on those who 
fail to do their jobs with respect for the risks others are taking.

But the space program as a whole is fixable, and we need it.
