It's pretty obvious this was a goof on Adam's part by just hitting reply rather than copying in the original sender's email address. This happens to everyone and there's no reason to call attention to it - I'm sure Adam is already embarrassed and perhaps a little concerned that his current employer might see the message. I don't understand what you mean by permissions. It has nothing to do with permissions, just a mistake that all of us make from time to time, to varying degrees of consequence...
