Project: HTML email with embedded survey form and related administrative features.
Time Frame: 1 week [due 5/10] On Site: Optional Description: 1.. 7-10 question survey with multiple choice, true/false, and free text answers. Along with some text fields for contact info (name, company, etc) 2.. Submitting the survey posts to SQL DB and provides a thank you screen. 3.. Admin area will display all fixed answer (t/f, multiple choice) result totals [ie. 50 True, 23 False] 4.. CF will also provide this data for a flash graphing tool for each question. 5.. Admin will be secured through directory security on NT (not in scope) 6.. Individual results will be listed in a csv text file to be generated dynamically by CF and available for download through the admin. Please respond if you are available to work on this project within the listed deadline as well as a *short* list of qualifications and related work.
