You will need to click on "CF Settings" to enter the rest of the required information. An example db that I have set up requried Data Source Name Database Name Server Host Name username password in order to get working. Also, if you are accessing mysql on another machine, you'll have to allow permission to access that database from that machine name and IP in mysql.
