Charlie, thank you for catching these problems.  All are in Appendix H, section H6.3.  I will deal with them.

We haven't decided whether to switch from the Trac system to the Github issue tracker, but as an experiment I will use the Github issue tracker for this one.  See https://github.com/cf-convention/cf-conventions/issues/110 .
