looking at the example you sent, it pretty obviously just looks like
name/value pairs. I would bet order is unimportant (since the fields
appear named)

you will need to build one for each variable needed in the ouput form,
and will need to change the /f (which I assume means file) to point to
your pdf form file.

all the rest would stay pretty much the same.
