We have not offered shared MX hosting but do have several dedicated boxes and the 
problems that we see are all related to the worries we have about shared hosting and 
the difficulty of fully separating the sites.

The standard ask we get from the dedicated clients is "can you clear the cache? we 
have got such-and-such a page stuck there". Normally we can sus out what the cached 
page is and delete it but on a shared environment even thinking about it is scary.....

I would like to see some sort of environment where MX "knows" about the virtual sites 
on the box and differentiates them in the server structures, say sub-folders in the 
classes directory, one for each site.

I am interested on what anyone else has to say in this area, I might be a tad out of 
touch :-)
