Perhaps the DNS for the domain name resolved to a different IP than
the one you're using manually, and consequently you're requests are
taking a different route to the server, and thereby arriving from a
different IP that isn't listed in the debug config as an IP to send
debugging information to?

Or perhaps you're connected to a different CF instance with the
name-based vhost than with the ip-based vhost?  One has debugging
enabled and one doesn't.
