Two possibilities I can contribute:

1) Under certain conditions, fields AFTER a form upload field don't get
passed when the form is submitted.

2) Under certain conditions, I have found that forms will not submit
properly if they are not carefully nested consistently in a table. IE:

<table>
<form>
<input type="hidden"...
<tr>
   <td>
    <input type="text"...
   </td>
   <input type="hidden"...
   </form>
</tr>
</table>

Move the closing form tag below the </tr> tag and it works. Can't remember
what platform it was on, and I can't explain it, but it happened.
