We use Anomaly Tracking System (ATS) from Microsoft. It was included free on Visual Studio 5. The version we use is a FoxPro application but allows you to create user defined fields and choose what to display and not display. Pretty simple.
I know there is a web based version that can be downloaded from Microsoft.
