I should have mentioned this before: I have checked the 'Check that file exists' application mapping setting for .cfm.
I notice that if my url is http://servf18, I get access denied, but if it is http://servf18/default.cfm, it works. Default.cfm is the file that runs be default, due to my IIS settings.
