Dave This type of error is usually caused by a Date/Time issue for cookie expiration.
this may be solved by a number of methods.. depending on the issue.. Ex: one time my user login script was doing exactly what yours is doing.. and it ended up.. the system clock was incorrect.. one of the admins had set the timezone of the server to their timezone at home.. and then another admin noticed the time was incorrect and set it to the correct time in a different time zone.... .. and the majority of the cookies expired immediately. Check your system clock.. and server system clock to make sure that it is correct..
