Stack the form fields.  Like this:

You can also use a style to apply a smaller font so the form will fit
into a 640x480 screen without any trouble whatsoever, even with a left
menu bar, like this:

And as was pointed out earlier, don't build 1024 forms, or you'll be
sorrrrry :D.
