Rick,

It's not consistent - and it's more prevelant on a cf 4.x box than a cf 5 box - but I've seen it several times on both.

  It's a known bug in 4.5 and 5.0. There never was a patch issued. Unless
  you count upgrading to MX a "patch" :-)
