I was wondering if anyone has heard of a Cold Fusion Based User Survey Tool/Form Generator that is robust enough to have a user fill out a survey online. It would need to be able to be dynamic so that based on an answer to the question, it will display a different question next. One of the applications I need this for is for a user to fill out a survery of their technical environment that will be approximatley 12 pages long. I currently have these in a Word Doc that I want to convert to a web based form.
Any suggestions would be appreciated!
