It's really FORM work flow process if we're talking about lots of different types of forms to be used by an organization.  Stuff to be captured/stored in the db, is a given.  Three things are critical:
1) Form template creation;
2) Form template use
3) Process of submitted forms (for instance who approves what?)
And I think some one has mentioned an iota part of the first process.

On top of that, and you have to be extremely flexible in design, otherwise, write once, use once, probably you want to avoid that.
