Donna, I have a client, in fact my wife works for them, and they do this for lawyers as they are a document requisition copy service (http://www.acrc-copy.com/).
In a nutshell, they scan the documents on a large Canon ImageRunner which saves the file as a TIFF, then they convert it to a PDF using Adobe Acrobat.
