I have used HTMLArea 3, and it is pretty good, but it is a pain in the ass to configure correctly.
By far the best HTML editor I have used is the newest version of the old Spectra editor, eWebeditPro. It supports CSS, XHTML, file uploading, even image manipulation. If you bu the +XML version, it allows you to configure XML templates where users only have certain writable fields inside the editor that they can change- like Dreamweaver templates only right inside the editor on the site. Really nice. Downside? They say it is cross-browser compatible, but I have never been able to get it to work with Firefox, so it is Windows/IE only in my apps. And oh yeah, it's not free or even cheap if you plan to use it with a lot of users.
