No system crashes. I did a directory scan for FOUND.* and nothing found.

Did you have any system crashes and have damaged files discovered in
scandisk?  See if there's a directory called "FOUND.???" (??? Being a
zero-padded number) that's date might correspond to when the error started
happening?
