No, it does not. The file is index.html and calls the include
inc_header.shtml. I wonder if that is the problem.

Jim McAtee wrote that, "If the page on which the SSI directive is included
has an extension of 'html' then that extension would have to be mapped to
the SSI engine ssinc.dll."

Since every page calls an include, does this mean that ALL of my pages
should have an .shtml extension?

Does the file containing the #include have .shtml extension? (The included
file can be any filetype).
