Ummm... haven't seen it.

I'm guessing it would be an invalid value, like an out-of-range value,
against an integer column... perhaps a non-numerical value trying to
insert into a number column? I've found that quoting values like this:
"insert table (column) ('1')" can result in odd responses. That's why
I've really embraced cqueryparam... no escaping, consistent typing,
blah blah blah.

It works pretty well... so maybe that's your answer.
