We too rarely have more than one person working on the same project. We
sometimes have the need to bring one person in to help another but those
times are rare as well. I have yet to work with a company that used some
form of source control. About 6 years ago I worked on a project where we did
implement CVS since there were three of us on the project. It never worked
100% but I'd imagine after so many years that many improvements have
happened.
As far as database changes, we have no real system for keeping schema
logins and passwords. We actually need something that a team could have easy
access to but none of the ideas they looked into seemed "secure" enough for
storing passwords.
