I had a similar situation about six months ago.  Looked all over and
couldn't find a cheaper or better way to do it than efax.  They wound
up opting for a printable form in the browser that they sent to a
printer and walked to a fax (!).  I'll bet when they grow in size
we'll revisit that solution.
