The error was caused by the following line at the start of the highest level
program:

<CFSETTING enablecfoutputonly="yes">

Not sure why this would cause the error, but when I removed the line,
everything worked fine.

Sounds like you've either got no XML content (perhaps you need
CFOUTPUT around the body of your include?), or you've got a tag that
isn't closed somewhere.
