I don't think we did anything special for CFFILE to work. What I do remember is that the web server has to have permissions to write to the Windows temp folder and that there was a bug in CFMX where none of the CFFILE temp files were deleted as they should be and the disk would eventually fill up.
Since you are getting a 500 server error I would suggest looking in the IIS and CF log files to see the error that is occurring.
