When you connect to your Exchange server using Outlook, you are using Microsoft's specific protocols to communicate. When you connect to Exchange from CF using CFMAIL, you're using SMTP. Your Exchange server needs to be configured to allow SMTP relaying from your CF server. Google "Exchange SMTP relay" to find out how to do this.
