Well, i guess for users who surf with javascript disabled behind a firewall or other technology that strips referrer information, you're left with tracking the page URL yourself and embedding it in every link and form by hand, as suggested by Mike T. Obviously, that won't work for links coming in from other sites you don't control. Or for people who just type in the URL. Plus it's a lot of work...
