I have  built a couple of multi-lingual sites (actually more than a
couple) and we use language translators (as in people) to translate
the content and its stored in the database (and cached)

apart from the content being stored in the db, I also developed a way
(which I am not sure if   it is available on CF4) to use .ini files,
so you have different .ini files for all the labels and buttons, for
every label I used (at the time) a custom tag (since I was in CF5) to
put all labels such as <cf_label
lang="#Session.langid#">Search</cf_label>. What this would do is look
for a translation for the "Search" in the (as an example) spanish .ini
file and if it didnt find it, it would just put "Search" but add the
key to the ini file.

I had another process that read these ini files and made a grid of all
the labels that needed translation etc. So it looked like an excel
spreadsheet for the translators to enter the label translations.

With regards to content, it was stored as a structure with each
attribute that needed to be mutli-lingual being a struct with a key
for each language.

A lot of the content management was based around Spectra ideas, and I
had some tags that were like <cf_content id="xyz" lang="ES"
rContent="r_sContent"/>

There were loads of things in there that grew out of necessity but at
least I hope this helps
