The actual code only has:
<script type="text/javascript" src="../includes/cfform.js"></script>

...because the server appears to have an incorrect or no mapping on it. 
This was the only way I could get my validation working.

CF is adding these two itself, which obviously ain't correct.

It's not working for me, nor for many of the client's users. Whatever 
changed/happened was about two weeks ago. All forms were working fine 
until then. I'm totally clueless at this point.
