Your JS is referencing an object that has to be within the <form></form>. I
remember chasing one of these for a while, and I believe the syntax of the
<form> tag was incomplete.

CF code is all processed server-side, and the only code that should be
returned to the browser is HTML and Javascript. So are you sure it's not
your Javascript?
