Perhaps the fact that the form has 3 elements all named accessCode might
cause that problem?

Good to hear!

As for the too few paramters thing this is odd indeed. There is clearly
nothing wrong with the syntax of the SQL you are sending. Looking around it
seems that using the wrong table and column names can cause this problem.
Other causes are to do with explicitly typing the parameters. If the column
and table names are definately correct, try using qcfqueryparam and find the
appropriate cfsqltype for the access field being referenced as
cf_sql_varchar clearly does not work.
