Turned on that my problem was NOT having ISAPI Extensions enabled. Most of the instructions I saw only said ISAP Filters.
So.. to summarize... add web server role Under roles, click on "Web Server" then make sure IIS 6 management compatibility, ISAPI Filters, and ISAPI extensions are installed, if not add them by clicking "Add Role Service".
