We've encountered an error with Flex 3 & ColdFusion 8 where the
/flex2gateway URL just randomly went unresponsive. Not returning errors,
nothing written in any logs anywhere, just would not pick up the phone.
Trying to load it in a browser just caused the browser to sit there at
"Waiting for [domain]...", while the Flex apps would just sit until giving
up with "Channel.Connect.Failed error NetConnection.Call.Failed". Restarting
the CF service resolved the problem, but obviously that is a highly
undesirable course of action on a production server.

Is this a known issue, is there a fix, or even a known cause?
