I think the point your both missing is that your PDF solution is still web based, as it still has to submit the data to the database over the web, so the person filling in the form still has to be online, the only way for it to not be web based is for the user to print out the form, fill it in with a pen and post it back using snail mail.
To do what you want you actually don't need to write 1 line of code, take a look at www.formassembly.com An online form builder that will allow you to build the forms you need and store the data in a database, all for free. There are some other solutions ou there, but formassembly is the most powerful one I have found.
