if you're using the old formurl2attributes custom tag, then you need to either specify 
the
full app url
    (form action=http://www.domain.com/index.cfm)
or
set the displaybase to "no" in the custom tag
    (<cf_formurl2attributes displaybase="no">)
or
get the new formurl2attributes tag!
