I've been pretty happy with http://www.crystaltech.com. It's got all of the above (although with SQL Server, the lowest plan is $50/month, so I'm not sure if you call that low-cost or not). They also have a reseller feature where they just discount all subsequent sites you host with them (each successive site has a bigger discount).
