We use CyberCash WebAuthorize - you run your own transaction server and there is a tag out there for connecting CF with WebAuthorize. It's very simple to setup and use, but the initial cost can be a bit painful - to the best of my knowledge it was around $5k for the WebAuthorize software. The documentation on this product sucks, so if you go that route, I'd be happy to pass along the code we use to connect.
