I recall hearing that if your error page is less than 2K (or some such number) in size then IE will show its own error page vs. yours. Perhaps that's the issue? Also there's an "advanced" setting in IE about showing "friendly" HTTP errors (vs. server errors which apparently are considered "un-friendly" :). With the setting set to not show friendly errors, the page size doesn't matter (it's set to "friendly" by default though).
