The strategy I have used in the past is the following: 1) Create two tables, one which contains a user_id corresponding to each person in your department and a second for storing survey answers
2) Send a link to the survey to each user containing the user_id 3) Delete each user_id as the user completes the survey; require the user to have a valid id when entering the survey.
