I am working on something similiar here - printinout out information onto 
medical forms - and think I got the easiest answer:)
Just print it out using plain html and style sheets to exactlly position 
the fields where they have to be on the form. In testing, it looks like it 
will work on many different printers. Much faster and easier than working 
with pdf or word!
