I've built a couple of them, but haven't published them -- they're not stand-alone apps... A friend of mine built this one: http://www.cfxtras.com/SalesComponentDetail.cfx?componentid=81 also cf_webwizard and cf_dataform, both also on the cfxtras website... I don't really know anything about them... Other than that, I'd say search the MM Exchange for "dynamic forms" or just "forms" and see what you come up with ...
