This has happened to me not to long ago, all was working fine for a while. Then I had the client who had never used the program, go to the url and it didn't work. I had changed the directory that the program had run in but forgot to update the code, but since the ActiveX control was still installed, it kept working on all the machines I had been testing on...doh! Are you 100% sure that the lpk file is in the right place? Try entering the url of the control into the browser... Also have you uninstalled the control and started fresh? Tools/Internet Options/Settings/View Objects/Right click/Remove
