Sleeping on the problem, I don't think this is the reason for the error. If
it's proxy related, all 3 (local,dev,staging) servers would have to pass
through the proxy. The problem is on the local and staging servers only. The
development server has worked fine.

That's why I thought it maybe a permissions problem or perhaps something
related?
