Yes! I've had huge amounts of problems with exactly what you've described. This was my single biggest problem with Dreamweaver that made me turn my back on it. It was frightening that changes you'd made weeks ago could disappear when someone else modified that file, because Dreamweaver insisted on using local copies of files on a shared development server, even when you've selected the option to directly modify remote files. Why give you this option and then completely ignore it? In my mind, its extremely bad when you can't trust your development tool to get files from where you've told it to get them from.
I've no ideas other than not using dreamweaver (use homesite+) and also use a version control system like CVS.
