CrystalTech provides a free shared SQL db with dedicated servers but they will allow you to install MS SQL Server 2000 on your dedicated server.  They also recently came out with a plan to allow use of MS SQL Server 2000 on a monthly plan ($225/month single CPU) rather than having to buy the full license upfront. From their website:

Microsoft SQL is an ideal database management solution for web-based applications. Unfortunately, it comes with a rather pricey overhead. CrystalTech makes it easy for designers and developers to bring the power of SQL to their dedicated servers by providing you with the ability to pay a monthly fee for a full standard, single CPU (no connection restrictions) license. This gives you the ability to experience the power of SQL without having to pay a large sum of money upfront. We will even install and configure SQL for you so you can be up and running in no time.

Hi, thanks for the replies, but I know if we want a hosting dedicated server for our site with www.crystaltech.com... it will be impossible to have SQL Server 2000 in our dedicated server... the SQL Server is shared... So we cannot be admistrator of SQL Server 2000... We don't like this because we want to control every thing... we want a dedicated server with ColdFusion and MS SQL Server 2000 that we can control totally... with www.crystaltech.com is not possible!... and with www.hostmysite.com... it can be possible...

As this question comes up a bit, Ill just be to the point. The majority of everyone seems to endorse www.crystaltech.com and no one has had anything bad to say about them.

Subject: [CFCDev] CF hosting provider?

Is anybody using hostmysite.com as a CF hosting provider? I have heard some good things about them and was hoping to get some opinions. I am actively looking for someone to host us. We are using CF, FR, Flash and Flash communication server along with SQL2000.

Appreciate your input. Thanks
