I have experienced something similar but I haven't had time to reproduce it.

I observed many times in January this year and the way It happened was:
~100 requests per second
php-fcgi with 10 workers
php-fcgi suddenly hangs and then the CPU usage on cherokee worker jupmps
to ~790% (on a dual 4-core server, 800% is maximum possible CPU usage)

I don't know if this has been fixed, but I was planning to reproduce it
with an openx with enough load and running 10 instances of a php
application that sleeps for more than 120 seconds until the PHP seems to
hang.

I got into a very demanding projecto so I have not had time to run this
experiment, but I will try it on March.
